早前香港政府向所有住戶派發『防疫服務包』，鼓勵市民在2022年4月8至10日進行自我快速抗原檢測。現在誠邀收過防疫包的市民，參加一項簡單不記名網上問卷調查。無論你有沒有做快速檢測，請你透過以下二維碼登入防疫普查平台，填答大約十個簡單問題，需時不超過五分鐘。

Population COVID-19 Survey
The Hong Kong SAR Government recommended members of the public to self-perform SARS-CoV-2 rapid antigen test (RAT) daily from 8 to 10 April 2022. We sincerely invite Hong Kong adult residents who have received the government’s “anti-epidemic service bags”, irrespective of whether testing had been performed during the 3 days, to complete a simple anonymous online survey, which takes no more than 5 minutes.

In collaboration with the Association of Hong Kong Nursing Staff, an online survey (in Chinese) is now conducted on practicing nurses to understand the coverage of influenza vaccination among nurses in the territory. Completion of the survey would entitle one to a HKD20 meal coupon. The deadline is 10 April 2016.
