Today, the midwifery community mourns the loss of one of our own.

Cayleigh Alston Cayleigh Alston of The Well Birth Services The Well Birth Services in TN was a great midwife; one of the women who chose to spend her life standing beside others at one of humanity’s most profound thresholds.

Her passing is a loss to her family, to the families she served, and to the profession she represented.

But it is also a painful reminder of something we don’t talk about enough:

😫Midwives die too young.😫

We spend our lives caring for everyone else. We answer the phone at midnight. We sit through long labors. We carry the weight of emergencies, loss, responsibility, and other people’s stories. We loose too much sleep. We pour ourselves into mothers, babies, families, students, and our communities.

And somewhere along the way, too many of us learn to treat our own exhaustion as simply part of the job.

It isn’t.

Midwives are not endlessly renewable resources. We are gentle human beings. We need rest. We need care. We need community. We need sustainable lives, not just sustainable practices.

Cayleigh inherited a legacy of midwifery from her great-grandmother, who caught babies in her own home. She carried that legacy into another generation.

Now those of us who remain carry it forward.

Let us honor the midwives we’ve lost by caring better for the midwives who are still here. Please call and check on your midwife friends today and regularly!

Green and Junior phases are about learning behind-the-scenes and coming out to do hands-on physical skills. But moving from Senior to Primary is an internal movement—a coming out of confidence and real-time leadership.

Here is the exact difference between these two final stages:

Senior Student: You understand the flow, take real responsibility, and run parts of care (prenatal, postpartum, lactation, and birth). But your judgment calls often happen out of sync—you realize what you could have said or done on the drive home or during your preceptor debrief.

Primary Student: You make judgment calls in the moment that your preceptor supports. You lead full-scope care, catch babies, and match your preceptor’s full on-call schedule—learning the true pacing and mental load of midwifery.

The mark of readiness for Primary isn’t just knowing what to do—it’s making the right call in real time.

Calling Postpartum Psychosis “rare” is dangerous. 🛑

When medical providers label a condition as “rare,” they mentally deprioritize it. They assume they will never see it in their own practice—which means they spend very little time learning how to recognize it. That is precisely how mothers like Aaisha end up interacting with FIVE different doctors who completely miss the warning signs.

The reality? Postpartum Psychosis occurs in 1 to 2 out of every 1,000 births—and that number doesn’t even account for the cases that follow miscarriages or pregnancy terminations. In a hospital delivering thousands of babies a year, this is an illness providers will encounter.

Tragedies like Lindsay Clancy’s case prove that we cannot afford to hide behind the word “rare” anymore. Every healthcare provider, midwife, and family member must be trained to recognize the signs. 🤍
