Rebuild the decision with the screen closed. Recognition creates a dangerous kind of fluency. A rationale can feel obvious because the mechanism, timing, and correct choice are all sitting in front of you.

Correct the beam angle before calling anatomy abnormal. A prospective study measured backscatter in patellar tendons as the beam moved from 0 to 20 degrees. The PERSEUS regional anesthesia guideline also supports ultrasound identification of anatomy and real time needle guidance.

D. Correct volume and chloride before anesthesia. Pyloromyotomy is urgent, but pyloric stenosis is a medical resuscitation problem before it is an anesthetic emergency. Repeated loss of gastric hydrochloric acid causes chloride depletion, volume contraction, and metabolic alkalosis.

Let units and physiology audit the equation. When you review an anesthesia formula, write the units beside every variable before you insert a number. Then cancel them line by line.

🧪⚡ A lower pKa often points toward faster onset. It does not let you predict the minute a block will be ready.

pKa predicts direction, not readiness. Alkalinization can increase the available base fraction, but the effect is not uniform across every drug and block. In a randomized double blind study of 180 patients, pH adjustment shortened onset in some epidural and peripheral blocks.

A. Reduce the pressure and time driving the leak. A human review supports lowering pressure, tidal volume, PEEP, inspiratory time, and rate as tolerated. In a five animal experiment, PEEP increased leak at every tested tidal volume.

To every SAA whose June 2026 CERT result was invalidated: I am genuinely sorry. You prepared, sat for the exam, waited for answers, and now have to prepare again because NCCAA determined that the June examination form was compromised.

NCCAA has been clear that this was not a finding against any individual candidate. I want MACPrep to be just as clear: if you are currently enrolled in an SAA program, sat for the June 2026 CERT, and received notice that your result was invalidated, I will provide complimentary lifetime MACPrep Premium access at no cost.

Classify the miss before choosing the repair. Most difficult misses fall into at least three useful categories: knowledge, discrimination, or sequencing. A knowledge miss means the rule was unavailable.
