2️⃣ Violation of isolation precautions
➡️ The sign clearly states: DO NOT ENTER WITHOUT PPE — this is a direct safety breach 🚫
3️⃣ High risk for infection transmission
➡️ This puts BOTH the nurse and patient at risk
4️⃣ Oxygen tank placed incorrectly
➡️ The tank is lying horizontally on the floor ❌
