CANCER AND EXERCISE- WORKING WITH PATIENTS WITH BONE METASTASES...

Evaluating patients and prescribing exercise programs for cancer patients with bone metastases can be challenging for both the patient and the exercise specialist.

Patients who have to battle cancer and manage bone metastases have a difficult journey. As a Cancer and exercise specialist I always take all 3 continuums into consideration when approaching this patient about exercise. In fact, I always take the 3 continuums into consideration when working with any patient, or client; the emotional, the cognitive or psychological and the physical.

I have found the emotional state of a patient to be the common denominator of everyone I’ve ever worked with.
The first thing that I do when I meet a new patient, or client, is to listen to them. You’d be surprised at what a cancer patient will tell an exercise person, especially one who is listening to them and not telling them what exercises they are going to do, how many times per week they are going to be asked to do them, how many reps and sets are in the program, or how good exercise is for them. Emotional first, cognitive second, and then the physical part is easy.

Once the person has opened up about how they feel about cancer, how they feel about exercising with cancer, how they think you feel about them exercising with cancer, how their support system feels about them exercising with cancer you can then explain to them that exercising with cancer is about helping to empower them to fight cancer, and perhaps do some exercises that will in fact make them feel better! It really does sound funny, but prescribing the exercises is the easy part.

I usually talk with the patient about how I view the affect that cancer has on the body and how it factors into their daily lives; and how and why I’d like for them to try to do some exercises. Once a person begins treatment for cancer their body and their mind changes. Their body now has 2 very specific jobs- to fight cancer and to receive treatment for cancer. Those 2 jobs use an enormous amount of energy- and now I am asking them to perform exercises that also require energy. (In general, people believe that doing exercise takes a lot of energy; hence the reason that so many people do not even attempt to exercise). It is now our job to empower these people to believe that YES, they can fight this cancer; YES, they can handle this treatment; and YES, they can walk a little bit and do a few exercises a couple of times per week; and YES, it will make them feel better and be more successful in their cancer fighting journey. I am very clear in explaining to these people that I have absolutely no idea how they feel, but that I believe in them.

The exercise prescription for patients with bone metastases is built on 4 components:

1)The emotional state of the patient (based on my interactions with them and their support system)

2)The functional status of the patient (based on functional testing)

3)The medical reports describing their bone disease

4)The exercise experience of the patient (based on their description of previous exercise and how I see them move)

The exercise program has 3 components:

1)Cardiorespiratory Strength and endurance

2)Muscular strength and endurance

3)Flexibility

The next step is to identify exercises that allow the patient to be successful in their program. One must remember to adapt the exercises as you cannot adapt the person.
