PBGCHS SPORTS HALL OF FAME NOMINATION PROCESS

Nominations may be self nomination or submitted by another person.
If by “another person”, please complete the appropriate form below and submit .
If self nomination, skip that form.
BOTH “other” and self nominations…..complete and submit the resume forms included below…OR… you may submit your own resume….BUT…ARE ENCOURAGED TO LOOK AT THE ENCLOSED RESUME FORMS TO INCLUDE THE INFORMATION REQUESTED ON THE GOOGLE FORM IN YOUR OWN RESUME…You want to submit a resume that includes/matches the criteria the HOF selection committee will be evaluating.
