Contraindications to ECMO include irreversible processes such as metastatic cancer, active bleeding, aortic insufficiency (for peripheral venoarterial only), and cases of contraindications to anticoagulation (aneurysms, bleeding disorders, and strokes).

Extracorporeal Membrane Oxygenation (ECMO): Considered for conditions that are severe but reversible. Two types: Venovenous (yellow) is indicated for respiratory failure (ARDS or COVID) and venoarterial (orange) is indicated for cardiopulmonary failure (massive PE, cardiogenic shock, ACS).

Left-Sided Ductal Dependent Congenital Heart Disease: Presents 1-2 weeks after ductus arteriosus closes w/ poor feeding, lethargy, irritability, and eventually shock. Hypoplastic left heart presents w/ cyanosis while critical aortic stenosis & coarctation of the aorta are acyanotic.

This week's topic: Candida vaginitis. Presents w/ thick white non-odorous discharge & vaginal itching. KOH wet mount will show pseudohyphae while pH will be normal (4.0-4.5). Risk factors = pregnancy, diabetes, OCPs, and recent Abx use.
