A non-restorable premolar fracture presents unique challenges for immediate implant placement and site preservation.

In this clinical case, oral surgeon Dr. David Ramos demonstrates a prosthetically driven approach using the Dentis Guide and an AXEL 4.2 × 12 mm implant, with simultaneous allograft bone grafting and immediate provisionalization.

The provisional restoration was kept completely out of occlusion to support healing and protect the implant during the initial phase.
