In order to be cleared on Final Forms for July 6th, the following items must be completed:
1. Parent Signatures
2. Student Signatures
3. IHSAA Health Questionnaire & Consent Form
4. New IHSAA Physical (If “yes” is selected on any of the questions on above form in number 3)

Reminder to all football players:
You only have a few days to get eligible for day 1 on July 6th! Keep working on those final forms, everything needs to be turned in for you to practice. We can’t wait to see you guys 🤘🏼
