Formula

👉 First day of Last Menstrual Period (LMP)
➕ Add 7 days
➖ Subtract 3 months
➕ Add 1 year

Important Points (NCLEX tip 🩺)
• Assumes a 28-day menstrual cycle
• Best used in women with regular cycles
• Accuracy decreases if cycles are irregular

Fundal height is the distance (in centimeters) from the top of the p***c symphysis to the top of the uterine fundus. It helps assess fetal growth and gestational age.

From 20–36 weeks:
Fundal height (cm) ≈ Gestational age (weeks) ± 2 cm
Examples:
• 24 weeks → ~24 cm
• 32 weeks → ~32 cm

Landmarks by Gestational Age
• 12 weeks: Just above p***c symphysis
• 20 weeks: At the level of the umbilicus
• 36 weeks: At the xiphoid process
• After 36 weeks: May decrease slightly due to lightening

GTPAL is an obstetric history shorthand used a lot in nursing exams (including NCLEX).

What GTPAL stands for:
• G – Gravida: Total number of times a woman has been pregnant (including current pregnancy)
• T – Term: Number of pregnancies delivered at ≥ 37 weeks
• P – Preterm: Deliveries between 20–36 weeks
• A – Abortions: Pregnancies ending before 20 weeks (spontaneous or induced)
• L – Living: Number of living children

Example:

G3 T1 P1 A0 L2

Means:
• Pregnant 3 times
• 1 term delivery
• 1 preterm delivery
• 0 abortions
• 2 living children

NCLEX tip 🧠:
• Multiple births (twins, triplets) count as ONE pregnancy for G, T, or P,
but each baby counts separately for L.
