I am having surgery on October 27th and need to have a week off to recover. I will be closed October 27th through November 3rd.

Also a reminder of upcoming Holidays I will be off

Hello! Just a few reminders and updates!

I have to close Friday August 13th to take care of medical appointments for my family

Please remember that I am closed August 23rd to August 27th.

I am sending out new enrollment forms to ensure I have everyone's most up to date information. Please return them by 7/23/2021. I am also including a form that you can fill out, and return back to me (I can fax it), so that I can get your child’s most up to date immunization records. I am required to have this info on file. Please return that with the new enrollment form. If you don’t want to fill out the immunization form I will need you to provide your child’s up to date immunization record by 7/23/2021.
