Physical Exams: The Provider Way ๐ฉ๐ฝโโ๏ธ
Nursing students and nurses, ever wonder how NPs do their exams? Bookmark this post and learn! โผ๏ธ

1. SUBJECTIVE DATA: This includes your chief complaint- what the patient came in for, history of present illness- a summary of the symptoms and characteristics of what the patient is experiencing. It also includes your medical history, allergies, current medications and illnesses, and social history. It also includes a review of systems where you ask about symptoms related to different body systems (presence or absence of symptoms).

2. OBJECTIVE DATA: This is your physical exam. It includes vital signs and all aspects of the head to toe assessment. This is normally what we do as nurses!

3. ASSESSMENT: This is our diagnosis or multiple diagnoses that we conclude for the patient. We put ICD 10 codes on each diagnosis.

4. PLAN: This is the treatment plan for the patient. This includes labs, radiology, tests, medications, referrals and education for the patient.

As nurse practitioners we need to ask A LOT of questions as we are diagnosing the patient. The history is one of the most important parts of our appointments with patients. ๐๐ก๐ฒ ๐๐ข๐ ๐ฒ๐จ๐ฎ ๐๐ก๐จ๐จ๐ฌ๐ ๐๐๐ ๐ง๐จ๐ญ ๐๐๐? I get this question all the time! Here are my reasons for choosing PNP:

1. Ability to work outpatient hours. I knew for a fact the hours you work as an NNP were not for me. I was never a fan of 3 12 hour shifts. I like having a set schedule every week and getting home before itโs too late.

2. Options to work in primary care. As a NICU RN I really really enjoyed having feeder/grower patients over high acuity. Thatโs when I knew I preferred to work with stable healthier kids over high acuity patients.

3. Flexibility to work with a variety of pediatric patients. As a PNP I have options to work in various outpatient pediatric settings. As an NNP Iโm limited only to NICU.

4. Less mental stress. The NICU can be stressful as a provider. Youโre intubating micropreemies, inserting central lines and dealing with complex conditions. I prefer to do more teaching/education and preventative care. To me, itโs less draining emotionally and mentally!

๐๐ฎ๐ซ๐ฌ๐ข๐ง๐ ๐๐จ๐ฎ๐ซ๐ง๐๐ฒ โจ So much has changed in my 3.5 years of being a nurse! Itโs amazing how many paths you can take in nursing.

1. Started college as a pre-med major. Would I have switched to nursing on my own if I wasnโt losing my mind in ochem? Probably not! Iโm so glad it prompted me to explore other careers in healthcare and pick this route. I blindly decided to choose pre med since my friends were. Iโm 1000% happier as a nurse. Does that mean I couldnโt have been smart enough to become a doctor? Absolutely not! Iโm doing essentially the same thing as the pediatricians Iโll be working with once I graduate as a PNP!

2. Got my CNA license once I became pre-nursing and started to work as a CNA. Worked in Burn ICU and Float Pool!

3. Graduated with my BSN and started off as a new graduate nurse in the NICU. Yes itโs 1000% possible to get a new grad job in a speciality like NICU. Donโt fear applying to those positions and donโt force yourself to work on a floor you have no interest for.

4. Moved to Dallas and worked in another NICU. This NICU was higher acuity than my first job. I learned so much about taking care of complex patients from working here. I also started NP school and this Instagram page a few months later!

5. Started to focus more on my page and quickly realized I had a passion for mentoring nursing students and nurses. My page started to grow a lot over quarantine. โข
6. Started my business! I would have never started it had quarantine not happened. With the growth of my page and increased demand for help from students and nurses, I recognized the need for my business.

7. Quit my bedside job after working for 3+ years in NICU. Finishing up NP school and focusing on growing my business.
โข
8. In 8 months, Iโll be graduating with my MSN. I still have so much left in my nursing path! Maybe Iโll get a DNP, Iโll find my first PNP job and maybe even teach nursing students on the side!

There is SO much you can do with your nursing degree. Donโt force yourself to stick to one area of nursing when you can grow in so many ways! I never saw myself doing so much in nursing. Comment below and let me know what your goals are! ๐๐ฝ

I didnโt know Nima but many people have been sharing this story. This is the note she left behind for her family. I think as a previous NICU RN and future pediatric NP this is SO important to talk about. Nima suffered from postpartum depression. 1 in 8 women experience postpartum depression according to the CDC. As a NICU nurse I always made it a point to ask moms how they were feeling. Especially since they had the added emotional stress of having a baby in the NICU, it was very very important to ensure that they were okay. As a PNP student, I always ask moms during infant checkups how theyโre feeling. Itโs so important to make sure they have resources and support. Having a baby is A LOT. Women go through so much during this time. Not only did they carry a baby for 9 whole months but also went through the physical and mental pain of having a whole human being coming out of them. Then you add the loss of sleep + hormones trying to regulate themselves after the baby is born. We need to bring more awareness to PPD. I urge you to donate to Postpartum support international (link in bio) and please check on moms! If youโre a NICU nurse, L&D nurse, postpartum nurse or working with moms who just had a baby in the past year, please check on them. Nimaโs story is one of many women who have lost the battle to PPD. We need to do more for these moms โค๏ธ

10,000 OF YOU โค๏ธ๐ญ If you wouldโve asked me even six months ago if I saw myself growing my page to this, I would have laughed at you! In early March I had under 1800 followers. I started this account September 2018 as an outlet to share my journey to becoming a nurse practitioner. Like anyone, I hoped to eventually grow a bigger audience but I never thought I could do it! It took me close to 2 years to get here. Itโs so easy these days to buy followers, but I really wanted an authentic audience. Iโm so thankful for all the growth this quarantine brought to my page! Not just to have โmore followersโ but because I can impact so many more of you. I am grateful it led me to creating a business and finding ways to help more and more people in their journey. ๐๐๐๐๐ ๐๐๐ ๐๐๐๐๐๐๐ ๐๐ฝ I remember being a pre-nursing student eagerly waiting to complete my CNA class! It was so exciting to learn how to take vital signs and work alongside nurses! I remember my first ever CNA job in the Burn ICU and how inspired I was by those nurses. The moment I got into nursing school, the day I was floated to NICU, graduating nursing school, passing NCLEX, starting my first day as a NICU RN...I recall each of these moments in my journey! Now, Iโm so close to becoming a PNP! One day, many years ago, I dreamed of reaching that goal of becoming a PNP and now Iโm here so close to it โค๏ธ Trust the process, you will get there!

๐๐ ๐ฏ๐ฌ ๐๐ ๐ฏ๐ฌ ๐๐? I get a lot of messages asking why I choose NP over the other routes. I canโt speak for any of those routes since I never did them but I can talk about why I chose the NP route.

1. Holistic care is the foundation of your medical treatment process. As nurses we are taught these fundamental ways of caring for the patient and when we become nurse practitioners, it translates into that.
โข
2. Building off your real life work experience. I canโt tell you how easy talking to parents comes to me because I worked as a NICU nurse. I know exactly how to interact with parents. My assessment skills and ability to recognize symptoms also comes from my work experiences.

3. Less debt. While you do NP school, you can continue to work as an RN. Although I quit now, I worked for majority of the program and have zero debt.
โข
4. Tuition reimbursement: Your hospital may even pay for part of your tuition. Who wants loans?! Nobody! Once again, another route to no debt.

5. Vast career opportunities: The best part about nursing is the fact that there is so many things you can do. Even as an NP you can teach in nursing schools or do research and advance your career further to get a doctorate! I love that I can switch to the education side when practicing in clinic doesnโt fit my lifestyle.
