The PSA test is not the problem. What happens after the result comes back is where things go wrong.

Getting a PSA test is smart and I support it. What I do not support is letting a urologist interpret it without question. Before you accept a referral that puts you on a conveyor belt toward biopsy, pause and ask whether watchful monitoring over the next few months is a reasonable alternative.

The prostate gland makes its own DHT internally, and your body saturates it at relatively normal testosterone levels.

Once your testosterone is above roughly 300, the prostate has already maxed out the DHT it can absorb. That means raising your testosterone from 400 to 1,200 through therapy produces no additional effect on the gland, no change in PSA, no increase in size, no accelerated growth.

If your surgeon told you the full truth before scheduling your prostatectomy, most men would walk out of that office.

Here is what genuine informed consent sounds like: "This surgery will not reduce your likelihood of dying from prostate cancer. It will guarantee some level of harm, infection risk, potential impotence, possible cardiac event. Now, when would you like to schedule?" That conversation is not happening.
