LAST Immediate Management — Airway, Benzodiazepine, Lipid Early
⚠️ Suspect LAST? Move quickly.
The priorities are straightforward: call for help, secure oxygenation and ventilation, control seizures, and get 20% lipid emulsion to the bedside.
Give 100% oxygen and avoid hypoventilation because hypoxemia and acidemia can amplify local anesthetic toxicity. Benzodiazepines are preferred for seizure control. Large doses of propofol should be avoided when significant cardiovascular instability is present.
Most importantly: serious LAST is an indication to begin lipid emulsion therapy early—don’t wait for cardiac arrest.
Every anesthesia location using local anesthetics should have a readily accessible LAST kit and cognitive aid.

LAST Prevention — Prevention Starts Before Injection
💉 The best LAST rescue is preventing LAST in the first place.
Bupivacaine carries particularly high cardiotoxic potential because of its strong and prolonged interaction with cardiac sodium channels. Prevention begins before the needle ever enters the patient.
Use the lowest effective local anesthetic dose, calculate the total dose carefully, aspirate before injection, inject incrementally, use ultrasound when appropriate, and account for patient-specific factors that may increase toxicity risk.
And remember: hypoxia and acidemia can worsen LAST.
A simple habit worth considering during the procedural time-out: say the planned local anesthetic, concentration, volume, and total milligram dose out loud.

1. LAST Recognition — Think LAST Early
🚨 Anesthesia Clinical Pearl: Local Anesthetic Systemic Toxicity
When neurologic or cardiovascular changes occur around the time of local anesthetic administration, LAST should be on the differential immediately.
CNS symptoms are often the first clue—agitation, perioral numbness, tinnitus, confusion, dysarthria, seizures, or decreased consciousness. Severe toxicity can progress to bradycardia, conduction disturbances, ventricular arrhythmias, hypotension, and cardiovascular collapse.
One of the most important pearls: LAST does not always happen immediately. Delayed presentations can occur after block completion, especially after larger-volume injections or tissue deposition.
Know the signs. Keep monitoring. Be prepared to act.
