Let's review the Anterior Drawer/Lachman Test – crucial assessments for ACL stability!
Here's how you do it, future NPs: With the patient lying down, bend the knee and stabilize the thigh with one hand. Grasp the proximal tibia with the other, then gently pull the tibia forward.
The key difference between the two tests is knee flexion:
👉 Anterior Drawer Test: Knee flexed to 90 degrees
👉 Lachman Test: Knee flexed to 20-30 degrees
You're looking for excessive anterior translation of the tibia compared to the femur, indicating a positive test for ACL instability!
But remember, a positive sign doesn’t confirm an ACL tear alone, so imaging like an MRI will be needed to confirm the diagnosis.

Here's how to use this tool as a nurse practitioner: Flex the knee and apply rotation (inward or outward) to the lower leg, then gradually extend the leg while maintaining the rotation.
