Forest Hills Adult Day Center is a center that takes care of your loved one who suffers from Dementi
Day Service Cost:
Half Day (up to 5 hours) $50.00
Full Day ( up to 8 hours) $65.00
Transportation: *Up to a 7 mile radius only*
One way = $20.00
Round trip = $30.00
We are participants in the Passport, Senior Options, Care Giver Grant & Ohio Health Plan insurance programs.
