EMPLOYEE OF THE MONTH NOMINATION FORM
1) ATTITUDE/CHARACTER:
Sariah comes to work every shift with a smile on her face and love in her heart that she freely gives away to all that she comes in contact with. She treats each and every person with dignity and respect and is kind, generous, and compassionate in all of her interactions. She never has a negative thing to say and maintains positivity and an upbeat disposition even in difficult, stressful, or undesirable situations.
2) CONCERN FOR OTHERS/COMPASSION/SERVANT HEART:
Sariah embodies our mission and is truly here to love and serve for all of the right reasons. It is evident that she views her position and time at The Heritage Home as a service and mission of love versus a job. Sariah goes the extra mile and exemplifies the meaning of a true servant's heart: always eager to take action and serve others with humility and diligence.
3) YEARS OF SERVICE/DEDICATION:
Sariah started at The Heritage Home as a new CNA in May of 2025. She was eager to learn and grow and quickly became a confident, extremely competent CNA as well as a resident, staff, and leadership favorite. She attended nursing school during the Fall and Spring semester of 2025/2026 but remained committed to her employment at The Heritage Home during her breaks. She has worked full-time hours again this summer and everyone has loved having her back and an integral part of our home once again.
Sariah is the first to commit to the least desired shifts and is always willing to work weekends, holidays, night shifts, and everything in between without hesitation or complaint. She is the first to volunteer to come in at the last minute or stay late without notice. She defines the term “team player.”
4 ) ENHANCES WORKPLACE AND LIVES OF OTHERS:
Sariah brings a joy and light to our home that co-workers, residents, and families can’t help but notice and have come to appreciate and yearn for. She maintains a positive, caring attitude in all circumstances. She is always striving to make life better for the residents and goes above and beyond to do all the “extras” such as taking them outside to enjoy the sunshine or serving them a special snack or facilitating an activity or craft with them. She has also been nicknamed “drill sergeant" definitely in the best way possible as she pushes residents to complete their exercises or wellness routine to enhance and optimize their health. She truly puts the residents first in all that she does and always has their best interest at heart.
5) EXHIBITS INITIATIVE/ENTHUSIASM BEYOND THE CALL OF DUTY:
Sariah shows up ready to work and positively contributes from the moment she arrives to the time she leaves. She has been described as the “energizer bunny” when a resident tried to portray that she is always working hard and doing so with enthusiasm and an energy that is unmatched. From her very first day of training, she made the transition easy. She never hesitates to jump in, takes initiative, holds herself and others to a high standard, and leads by example.
Sariah has blessed our home and those that live and work here in more ways than one can count. She is extremely deserving of this honor and all the accolades the world has to offer.
