This paper asks a question with relevance beyond Indonesia. How do older adults in resource-limited settings seek care when family support is limited and uncertain? What role can community relationships play? Using nationally representative data from the Indonesia Family Life Survey, researchers identify four health-seeking profiles among older Indonesians: Limited, Basic Routine, Traditional, and Comprehensive. The findings show that spousal absence is linked to more limited routine health-seeking, while parent–child disconnection is associated with more comprehensive health-seeking among some older adults. Community trust may partially compensate for weak family ties, but community engagement is not a simple substitute for family support. The broader lesson is that health systems in Indonesia and other LMICs should not assume that spouses or adult children are always available to help older adults navigate care. Community-based outreach, health literacy, and culturally grounded interventions will become increasingly important as populations age and family networks change.

Contemporary Perspectives in Family Research (CPFR) is inviting manuscript submissions for a forthcoming special volume titled "Declining Fertility and the Devaluation of Childbearing".

Co-edited by CFPR Co-Director A/Prof Zheng Mu, Dr. Veronica L. Gregorio and A/Prof Sampson Lee Blair, the volume welcomes empirical, theoretical, practice and policy-oriented contributions on declining fertility, childbearing, family change and shifting norms around parenthood.
