π¦· Can adding buprenorphine to an infraorbital nerve block extend analgesia beyond 48 hours in canine dental patients?
A recent randomised crossover study in dogs compared a standard bupivacaine block versus a buprenorphine-augmented block over 5 days of MAC monitoring. The headline result was P=0.775 β technically "not significant." But the data tells a more nuanced story.
The key findings:
π At 48h: 50% of buprenorphine dogs still had reduced MAC vs 25% of bupivacaine-only dogs
β± At 96h: bupivacaine effect was ZERO in all subjects β 25% of buprenorphine dogs were STILL covered
π‘ Bupivacaine alone lasted 24β72h β far longer than the textbook 4β8h
Why did it "fail"?
β N=8 (needed ~40 per group for power)
β Healthy subjects = minimal peripheral mu receptor upregulation
β Supramaximal dolorimetry may mask subtle analgesic effects
Bottom line: statistical insignificance β clinical irrelevance. The adjunct is safest candidates are patients with chronic periodontal or oral inflammatory disease β where peripheral mu receptors are upregulated and most likely to respond.
Haemodynamics? Statistically significant HR and MAP trends β but all within acceptable clinical limits. Safe protocol.

Patient safety during anaesthesia starts with choosing the right equipment. Body weight is everything β here's a quick reminder:
Breathing Circuits:
< 3 kg β Neonatal Mapleson D
3β10 kg β Small circular circuit
> 10 kg β Large circular circuit
Always remember:
β
Match the rebreathing bag to your patient (3β5 tidal volumes)
β
Always perform a leak test before every procedure
β
Set fresh gas flow (FGF) correctly for the circuit type
β
Keep dead space in check β max. 20% of the patient's tidal volume
