A caesarean is one of the few procedures where a single anaesthetic has to keep several patients safe at once. Almost every drug given to the mother crosses the placenta and reaches the neonates within minutes — so the whole plan is built around drugs that are short-acting and reversible, timing them carefully around delivery, and keeping the interval between induction and delivery as short as possible. And when a newborn is slow to start, a clear airway, warmth, gentle stimulation and — where opioids are involved — a single drop of naloxone under the tongue can make all the difference.
