⚠️ A small neglected lesion can become life-threatening.
A 67-year-old woman presented with progressive jaundice and severe weight loss. Further evaluation revealed a 5-year history of an untreated pigmented lesion on the sole of her foot.
🔍 Clinical Findings
An irregular 1.5 × 1.2 cm brown-to-black lesion was observed on the plantar surface.
🩺 Investigations
CT imaging showed:
• Large para-aortic retroperitoneal mass
• Liver lesions suggestive of metastases
• Enlarged adrenal glands
A Trucut biopsy confirmed metastatic malignant melanoma
(S100+, HMB-45+, SOX10+).
💉 Management Challenges
Due to financial limitations, advanced molecular testing (BRAF V600) and combination immunotherapy were not feasible. The patient received Nivolumab monotherapy and supportive care but was unfortunately lost to follow-up.
📌 Why This Case Matters
• A neglected plantar lesion progressed to widespread metastatic disease
• Early recognition and excision of suspicious lesions can save lives
• Metastatic melanoma management is highly complex and costly
• Early diagnosis remains the most accessible and effective strategy
• Routine examination of soles, palms, and nail beds is essential, especially in underserved populations
