Our today's case is from Synovial Fluid of right knee for an 55 years male from Ortho clinic, patient came with knee swelling, sample was bloody and viscosity was high, slides prepared and it was filled with Neutrophils as majority of the cells, but the important finding was the CPPD crystals that we found clearly, also few Corricosteroid crystals (light blue color)

Calcium pyrophosphate deposition disease (CPPD) is a crystal deposition arthropathy involving the synovial and periarticular tissues. Its clinical presentation may range from being asymptomatic to acute or chronic inflammatory arthritis. A buildup of calcium pyrophosphate (CPP) in your affected joints causes pseudogout. The extra CPP forms small crystals that build up in the cartilage and fluid-filled synovial membranes cushioning your joints. Eventually, the CPP crystals clump together inside your joints and cause a flare of pseudogout symptoms. It is unknown why CPP crystals form. Excess iron or calcium, or low magnesium, and an abnormal thyroid gland may be contributing factors. Joint aspiration is the process of removing some fluid from the sore joint(s). This not only helps your doctor determine if you have the disease, but it also removes many of the CPPD crystals that cause the pain and swelling. Removing the crystals also can help medications work better.

Some notes about " Proficiency Testing":

PT: involves testing samples with a quantity unknown to the laboratory and then comparing these results across laboratories that use the similar methodology.

Proficiency Samples should be measured in the same manner as patient samples.

Proficiency Samples can't be sent to reference laboratories for result correlation.

An unsuccessful participation or performance in PT means repeated unsatisfactory test scores for an analyte (or) for the entire testing event on;

* (2) consecutive PT events

* (or) 2 of 3 testing events

An unsatisfactory performance in PT means:

* failure to attain the minimum satisfactory score for an analyte in a testing event.

* Errors in entering PT results leads to incorrect results and also unsatisfactory performance.

* Failure to report results in the allowed time leads to score 0

If the lab receives a non-passing score, the lab must:

* investigate the cause of error

* implement and document corrective actions

Todays' Case

cold agglutinin disease

In the presence of cold agglutinin, a low temperature induces RBC agglutination, and an automated CBC analyzer shows decreases in RBC and hematocrit. Hemoglobin concentration is not affected by cold agglutinins, and MCH and MCHC are therefore markedly increased.

A common complaint among patients with cold agglutinin disease is painful fingers and toes with purplish discoloration associated with cold exposure

the effect of cold agglutinin on CBC results may be prevented if the blood drawn from the patient is immediately transferred to the laboratory without exposure to cold. Or incubate the sample at 37c and run it immediatly.

He was thirsty and hungry from three days so his father Imam Hussain A.S. took him outside and asked for water for his very young son, as answer Hurmala shoot the arrow which was used to kill huge animals. There are 3 riwayat about where did the arrow hit, Ali Asghar's head, neck or ear.All 3 of them are true because the arrow was bigger than Ali asghar's whole body.
