Effect of Intra-articular Platelet-Rich Plasma vs Placebo on Pain and Cartilage Volume in Knee Osteoarthritis 01/12/2021

Platelet-Rich Plasma Injections No Better Than Placebo for Reducing Knee Pain

Injections of platelet-rich plasma (PRP) are no better than placebo for helping reduce knee pain in people with knee joint osteoarthritis, according to a study published in JAMA.

Findings from the RESTORE trial, which included 288 adults aged 50 years and older with mild to moderate radiographic knee osteoarthritis, found no significant changes in knee pain scores or change in medial tibial cartilage volume at 12 months between patients who received PRP injections compared with patients who received a placebo injections.

“We conducted the study because there is currently limited high-quality evidence about whether or not platelet-rich plasma injections have benefits for people with painful knee osteoarthritis,” said Kim Bennell, MD, University of Melbourne, Melbourne, Australia. “Our results do not support the use of platelet-rich plasma for people with mild to moderate knee osteoarthritis.”

The study included 288 patients aged 50 years or older with symptomatic medial knee osteoarthritis. The 12-month follow-up was completed on July 22, 2020. Interventions involved 3 intra-articular injections at weekly intervals of either leukocyte-poor PRP using a commercially available product (n = 144) or saline placebo (n = 144).

The 2 primary outcomes were 12-month change in overall average knee pain scores (11-point scale; range, 0-10, with higher scores indicating worse pain; minimum clinically important difference of 1.😎, and percentage change in medial tibial cartilage volume as assessed by MRI.

Among 288 patients who were randomised, 269 (93%) completed the trial. In both groups, 140 (97%) received all 3 injections. After 12 months, treatment with PRP versus placebo injection resulted in a mean change in knee pain scores of -2.1 versus -1.8 points, respectively (P = .17). The mean change in medial tibial cartilage volume was -1.4% versus -1.2%, respectively (P = .81). Of 31 prespecified secondary outcomes, 29 showed no significant between-group differences.

“Pain in osteoarthritis is a substantial unmet need, so identifying new therapies capable of improving the symptoms and structure are urgently needed,” said David Hunter, MD, University of Sydney, Sydney, Australia. “Unfortunately, the particular treatment trialled in this study, PRP, whilst widely used and typically expensive, appears to be ineffective compared to an inert comparator.”
