Initial Requirements for Screening
Applicants must meet the following qualifications:

✅ 30% or higher College Admission Test (CAT) Result
✅ 85% or higher Senior High School General Weighted Average (GWA)

For Senior High School Graduates
• Original and Photocopy of Report Card/Form 138 (85% GWA or higher)
• Original and Photocopy of Good Moral Character Certificate
• Photocopy of PSA Birth Certificate
• Copy of College Admission Test (CAT) Results (Average must be 30% or higher)
• One (1) 2x2 ID Picture (White Background with Name Tag)
• Accomplished Interview Form (Program Level)

For Transferees
• Photocopy of Transcript of Records (TOR) and Honorable Dismissal
• Evaluated Grades from the University Registrar
• Photocopy of PSA Birth Certificate
