Provided below is a flowchart on how to request access to SGC documents for easy reference.

𝗦𝗧𝗘𝗣𝗦 𝗔𝗡𝗗 𝗥𝗘𝗤𝗨𝗜𝗥𝗘𝗠𝗘𝗡𝗧𝗦 𝗧𝗢 𝗔𝗖𝗖𝗘𝗦𝗦 𝗦𝗚𝗖 𝗗𝗢𝗖𝗨𝗠𝗘𝗡𝗧𝗦

As part its function as feedback mechanism, the SPNHS School Governance Council (SGC) would like to share its information policy regarding access to SGC Documents such as Minutes of Meetings, SGC Resolutions. The said policy is in accordance with the DepEd Order 22 s. 2022 (𝘐𝘮𝘱𝘭𝘦𝘮𝘦𝘯𝘵𝘪𝘯𝘨 𝘎𝘶𝘪𝘥𝘦𝘭𝘪𝘯𝘦𝘴 𝘰𝘯 𝘵𝘩𝘦 𝘌𝘴𝘵𝘢𝘣𝘭𝘪𝘴𝘩𝘮𝘦𝘯𝘵 𝘰𝘧 𝘚𝘤𝘩𝘰𝘰𝘭 𝘎𝘰𝘷𝘦𝘳𝘯𝘢𝘯𝘤𝘦 𝘊𝘰𝘶𝘯𝘤𝘪𝘭) and the SSG Resolution no. 1 s. 2023 (𝘙𝘦𝘴𝘰𝘭𝘶𝘵𝘪𝘰𝘯 𝘰𝘯 𝘈𝘤𝘤𝘦𝘴𝘴 𝘵𝘰 𝘚𝘎𝘊 𝘐𝘯𝘧𝘰𝘳𝘮𝘢𝘵𝘪𝘰𝘯 𝘢𝘯𝘥 𝘌𝘴𝘵𝘢𝘣𝘭𝘪𝘴𝘩𝘮𝘦𝘯𝘵 𝘰𝘧 𝘏𝘦𝘭𝘱 𝘋𝘦𝘴𝘬 𝘢𝘯𝘥 𝘈𝘴𝘴𝘪𝘴𝘵𝘢𝘯𝘤𝘦)

𝗦𝘁𝗲𝗽𝘀 𝘁𝗼 𝗔𝗰𝗰𝗲𝘀𝘀 𝗦𝗚𝗖 𝗗𝗼𝗰𝘂𝗺𝗲𝗻𝘁𝘀

1. Accomplish the request slip and submit a request letter to the SGC Elected Secretary, namely, Madam Jenny D. Espinosa in-person or online through the following link: http://tinyurl.com/4cs7f9w8
2. The SGC Secretary shall review and validate the merits of the request
3. Upon review and validation, the Secretary shall forward the request to the SGC Elected Co-Chairperson and Designated Co-Chairperson for approval.
4. The School Head shall be notified with the approved request for the final approval.
5. The requester will be contacted by the Secretary regarding the status of his/her request. If approved, s/he will be given a date for the pick-up of documents or the sending of its digital copies as s/he requested.
