HSE incident investigation:
Here is an exclusive footage from an event at an industrial site involving a seemingly basic and common task: wiping up a spill.
The scene began with a normal operation: an automated hoist tilting a chemical drum to empty its contents into a huge plastic container (IBC tote), with a worker in protective coveralls leant over directly at the pour point. During the transfer, some liquid spilt, which the worker wiped up.
Within half a second, the entire frame became a dense white cloud that covered the surroundings and remained for dozens of seconds.
What happened here?
