✅Photocopy of PSA Birth Certificate / Live Birth / Baptismal Certificate
✅ Must be 5 years old on or before August 31, 2026
✅ For children turning 5 years old from November 1 – December 31, 2026:
📄 Certificate of Completion from Daycare
✅ Basic Enrollment Form

👧🧒 Please bring the child during enrollment.
