For Senior High School Graduates and Incoming First-Year Students

• Two (2) copies of 2×2 ID Picture (white background)
• BukSU CAT Report of Rating (ROR)
• Original Form 138 (Report Card)
• Original PSA Certificate of Live Birth
• Certificate of Good Moral Character
• PSA Marriage Certificate (if applicable)
• Medical Certificate

For Transferees

• Two (2) copies of 2×2 ID Picture (white background)
• BukSU CAT Report of Rating (ROR)
• Honorable Dismissal
• Accomplished Subject Equivalency Form
• Original PSA Certificate of Live Birth
• Certificate of Good Moral Character
• PSA Marriage Certificate (if applicable)
• Medical Certificate

For Continuing Students

• Grade Slip
• Grade Completion Form (for students with INC or incomplete grades)
• Prospectus

Additional Requirements (If Applicable)

• 4Ps or UCT Identification Card
• Solo Parent ID
• PWD ID
• Certificate of Indigency
• Indigenous Peoples (IP) Certificate

All students are required to submit a Medical Certificate issued by a Government Physician. The certificate must clearly state that the student is “𝐏𝐇𝐘𝐒𝐈𝐂𝐀𝐋𝐋𝐘 𝐅𝐈𝐓 𝐅𝐎𝐑 𝐎𝐅𝐅-𝐂𝐀𝐌𝐏𝐔𝐒 𝐀𝐍𝐃 𝐈𝐍-𝐂𝐀𝐌𝐏𝐔𝐒 𝐀𝐂𝐓𝐈𝐕𝐈𝐓𝐈𝐄𝐒.”
