All concerned applicants are advised to complete and submit the necessary requirements on or before the stated deadline.

🗓️Deadline of Submission: On or before May 12, 2026
📍Where to Submit: Office of Admission and Scholarship (beside the Cashier’s Office)

Please submit the original copies of the following requirements:
1. Availment of TES-3B Form: https://bit.ly/REQUESTFORAVAILMENTOFTES-3B
2. Print two (2) copies of the Electronic Official Receipt (eOR) from PRC
3. Original Official Receipts for Allowable Expenses, which may include:
* Notary Public – notarial fees
* Review Center – review fees
* Insurance Company – insurance premium fees
* HEI, PSA, BIR, NBI – issuance of Transcript of Records, Birth Certificate, Marriage Certificate (if applicable), Documentary Stamps, NBI Clearance, Certificate of Good Moral Character
* Other entities/institutions – expenses necessary to comply with licensure examination requirements.
