PSA Birth Certificate
SF9 / Form 138 (SHS Report Card)
Proof of Income (choose 1):
• BIR Tax Exemption / Non-Filer
• DSWD / 4Ps Certificate
• Latest ITR of parents/guardian
• OFW / Seafarer income proof
Enrollment Form (to follow if not yet available)
Signed Parent/Guardian Certification
CTC of Form 138 (for graduates before SY 2015–2016)

Ang 2026 JLSS ay isang scholarship program ng DOST-SEI na nagbibigay ng financial assistance sa mga karapat-dapat na incoming third year college students (AY 2026-2027) na kumukuha ng priority Science and Technology (S&T) programs.

📌 Sino ang maaaring mag-apply?
✔ Regular second-year college students (AY 2025–2026)
✔ Enrolled sa DOST-SEI priority S&T courses
✔ May GWA na hindi bababa sa 83% o katumbas nito
✔ May good moral character at nasa mabuting kalusugan

✅ Maayos ang pagkaka-fill out ng forms
✅ At naipasa na ang application
