Please be informed that the CHED TES Scholarship Form has been updated. All concerned students are required to accomplish the latest version of the form.

📌 Instructions:

- Download or access the updated CHED TES Form through the link provided below.
- Ensure that all information is complete and accurate before submission.
- Forms submitted using the previous version will not be accepted but ALL APPLICANTS who submitted their application on AUGUST 3, 2026, are already in the database. There is no need to reapply or fill out this Google Form again.
