1. Visit the Schools Division Office-Quezon City (SDO-QC) located at Nueva Ecija St., Bago Bantay, Quezon City.
2. Request for your Form 137/ SF10.
3. Wait for your schedule to pick up the document.

Records Section Office is open on Mondays to Fridays except holidays from 8am to 5pm.
