Please be informed of the following enrollment schedules, requirements, and procedures.

SCHEDULE OF ENROLLMENT:

🟣 Kinder, Grade 1- Grade 6:
🗓August 14 - August 25, 2023
@ 8:00 am to 4:00 pm only

📋REQUIREMENTS FOR ENROLLMENT:

✅ Incoming Kindergarten
*Photocopy of PSA Birth Certificate
*5 years old on or before October 2023

✅For Grade 1 - 6
(Old students)
*School Form (SF9) / Report Card
*PSA Birth Certificate (if not submitted)
*ECCD (for incoming grade 1)

✅For Transferees:
1. School Form (SF9) / Report Card
2. Photocopy of PSA Birth Certificate
3. ECCD - Early Childhood Care and Development Checklist (Kinder record from the originating school)

❇️Procedures of Enrollment❇️

1. Undergo the Brigada Eskwela 2023
*Sign the attendance
2. Get the Basic Education Enrollment Form from the class adviser
3. Fill out the Basic Education Enrollment Form legibly.
4. Submit your requirements to the respective Grade level adviser.
a. Accomplished Basic Education Enrollment Form
b. School Form 9 (SF 9)/ Report Card
c. PSA Birth Certificate (Kinder and Transferee only)
d. ECCD (Grade 1 and Transferee only)
