For all 1st Year BSA students who will be taking the Skills Development (Practicum) subject this midyear, please be informed that the release of Enrollment/Registration Forms will start on May 26, 2026 until May 29, 2026, from 8:30 AM to 4:00 PM at the CANR SC Office.

For those who have not yet submitted their requirements, please submit the following:
• Parent Consent Form (authenticated and notarized)
• Medical Certificate

Students who have already submitted their requirements only need to verify their names in the logbook and claim their Enrollment Form.

The Office of Student Affairs and Services (OSAS) is now officially accepting applications for the Non-Academic Awards for AY 2025-2026. These awards recognize achievements in leadership, service, character, and extracurricular involvement that enhance our campus environment.

🌟 AWARD CATEGORIES
1. Leadership Awards
• Presidential Leadership Award: The highest leadership honor, with only one recipient for the entire University.
• Institutional Leadership Award: One awardee per campus for SSC officers with at least two cumulative years of service.
• Student Leadership Award: One awardee per campus for officers of any accredited organization with at least three cumulative years of service.

To ensure your application is processed, please strictly follow these formatting guidelines:
• Document Size: All documents must be printed on A4 size paper.
• Binding: Submissions must be ring-bound with a PVC cover.
• Color Coding:
o 🔵 Blue Cover: For all Leadership Award applications.
o 🟡 Yellow Cover: For all Service Award applications.
• Supporting Documents: Include photocopies of evidence (certificates, endorsements, activity proposals, etc.) to substantiate your achievements.
