1. 2 pcs 1X1 ID white background
2. 2 pcs passport size ID with nametag, white background and formal attire

4. 3 Photocopies of TOR/FORM 137/ FORM 138
5. 6 Photocopies of 1 Valid ID with 3 original signatures (per copy)
6. 1 copy of Updated Resume
