please be advised that every department should come to their scheduled date from 8AM-12NN and 1:00-6:00 PM only! STRICTLY FOLLOW YOUR ASSIGNED SCHEDULES!!

REQUIREMENTS:
• Official application form, provided by the AUSSC. (per department po yung pagpasa nito)
• Parent’s notarized waiver, ID of your parent (kapag above 18, you can sign your own waiver, however, make sure to attach photocopy of your valid ID with signature and your birth cert)
• Photocopy of Registration form (2nd semester) and ID.
• Medical Certificate
• Students code of conduct, provided by the AUSSC.
