• any of the ff proof of Educational attainment (High School/College) DIPLOMA/TRANSCRIPT OF RECORD/CLASS CARD/COC/ALS rating card

• photocopy of birth certificate

• 3pcs each: 2x2, 1x1 and passport size picture with collar and white background. MUST BE STUDIO SHOOTS and TAKEN WITHIN THE LAST 6 MONTHS
