1. Candidate must be a graduating student in Colegio De Muntinlupa;
2. Must have a cumulative Grade Weighted Average (GWA) of 2.50;
3. Must be an officer of at least one (1) recognized organization on campus for two (2) years;
4. Must have actively participated in the school activities during his/her stay in the institution – school events, community development (NSTP, LTS, ROTC), school conference and award, classroom programs, etc.
5. Possess good moral character and must not have committed any major offense, as certified by the Office of the Prefect of Discipline.
𝑹𝒆𝒒𝒖𝒊𝒓𝒆𝒎𝒆𝒏𝒕𝒔: Pertinent documents must be fastened in a long, white folder with earmarks and arranged in order listed below:

𝐑𝐄𝐌𝐈𝐍𝐃𝐄𝐑
We have removed the time selection in the Google Form to allow everyone to render their service at their most convenient time. However, please be reminded that the minimum required service duration is still 4 hours per day.
This does not necessarily mean that you need to be free for the entire day. You only need to ensure that you have no classes during the time you plan to render your service.
