Greetings! We are Sociology students from the Polytechnic University of the Philippines, Manila. This form is intended for participants who are willing to participate in our studies.

‼️Your participation is entirely voluntary. You can choose one or more.

The purpose of these research is to understand consumer preferences, experiences, and perceptions regarding various topics. All information collected will be kept confidential and will be used for academic purposes only.
