Please be advised that today (October 30, Thursday) is
the last day of Advising for all CSD students with
Academic Problems. All necessary documents (corrected,
verified and approved) by the CSD Faculty Adviser and the
Chairperson must be submitted today (preferrably BEFORE
the END of the OFFICE HOUR).
For strict compliance.

Please be informed that the Pre-Advising for all CSD students with Academic Problems in preparation for the Upcoming Enrolment on October 28, 2014 (Tue) is now on-going. You can come to school starting Oct 20-Oct 24 (Mon-Fri) and on Oct 27 (Mon).

In line with the said advising please prepare all necessary documents (if needed) such as the ff.:

1. Request Letter of Intent addressed to the CET Dean (Engr. Clydelle M. Rondaris thru the Department Chairperson (Prof. Leisyl M. Ocampo)
2.Study Plan (checked and verified by the College offering the program)
3. Checklist (c/o Department Chairperson)
4. Complete list of grades or complete grades thru E-Grades (GWA computed, PE and NSTP are not included in the computation)
5. Notarized Special Note of Undertaking (please make sure that it is signed by the Dean before having it notarized)

In line with the said advising please prepare as early as today all necessary documents (if needed) such as the ff.: Letter of Reconsideration, Checklist (with verified E-Grades), Prospectus, Study Plan.
Lastly, the Academic Head (Prof. Leisyl M. Ocampo, Chairperson) will be entertaining your queries in connection to your academic problems.
