A 60-year-old male is undergoing his routine hemodialysis session. Approximately 45 minutes into the treatment, the patient complains of dizziness, nausea, and generalized weakness. You assess the following:
BP: 82/54 mmHg (baseline was 138/76 mmHg pre-dialysis)
HR: 96 bpm
Skin is pale and clammy
AV fistula remains patent with a palpable thrill
You are the nurse responsible for monitoring the patient during the dialysis session.
Question:
What is the most appropriate nursing intervention at this time?
A. Place the patient in Trendelenburg position and notify the dialysis provider.
B. Stop dialysis and clamp the fistula immediately.
C. Increase the ultrafiltration rate to remove fluid more quickly.
D. Encourage the patient to ambulate to improve circulation.
