In support of the Commission’s commitment under the ACHIEVE Agenda to promote graduate employability, responsiveness of higher education programs, and evidence-based policy development, requesting to please fill in the CHED SCHOLARS/GRANTS TRACER SURVEY FORM to graduates who were beneficiaries of CHED scholarship and student financial assistance programs granted during Award Year 2019 and 2021, hence, the respondents of this survey are the graduates from AY 2022-2023 to AY 2025-2026. RESPONSES WILL BE ACCEPTED UNTIL JULY 31, 2026.

The tracer survey aims to gather relevant data on the employment status, career pathways, further studies, and professional experiences of CHED scholar-graduates. Specifically, the survey seeks to determine the alignment and matching of graduates’ acquired competencies and skills with current labor market demands and employment opportunities. The results of the survey shall provide valuable inputs in assessing the effectiveness and relevance of scholarship programs, improving policy directions, and strengthening the linkage between higher education and workforce requirements.

CHED SCHOLAR GRADUATES TRACER This form is used to monitor and record the graduation details of CHED scholars. The information collected will help track the completion of scholars’ academic programs and support CHED in evaluating the outcomes and impact of its scholarship programs.

Please refer to the list of qualified grantees posted below!!!

Requirements for Personal Claiming:
✅ Three (3) colored photocopies of your validated School ID
✅ Each copy must have three (3) signatures below the ID image

For grantees who are unable to personally claim their stipend, representative-claimants must present and submit the following documents:
1. Three (3) Original Copies of a Signed Authorization Letter from the grantee...
📌 Note: Only the nearest kin are allowed to claim on behalf of the grantee.
If the nearest kin are not available, another person may claim provided that a Special Power of Attorney (SPA) is submitted.
2. Grantee’s Validated School ID - 3 colored photocopies with three (3) signatures below the ID image
3. Authorized Representative’s Government-issued ID – 3 colored photocopies with three (3) signatures below the ID image

All signatures on the submitted documents must be written using a blue ballpen.
Please make sure that all requirements are complete, legible, and properly signed to ensure smooth processing of the stipend release.
