𝗡𝗼𝘁𝗲: In compliance with CTU Memorandum No. PJ06-311, s. 2025, the surrender of old student IDs for those who already graduated is MANDATORY at the SAO Office. Lost or misplaced IDs must be supported with an Affidavit of Loss.
