The LGU Sumilao is conducting a quick survey to gather data on how many Senior High School and College Studentsare residing in Sumilao for an upcoming educational assistance program. This initiative aims to provide support with your school fees and is being offered through the LGU Sumilao.

1 The Regional Office will handle the screening process. If you have already received educational assistance this year, there is a chance that your name might not appear on the final list. The main reason why names are removed from the final list is if the student or their guardian has already received financial assistance from the LGU for this year. If you or your guardian has already availed of assistance, there is a high chance you wonโt be included in the final list. However, as long as you fill out the form, there is still a possibility that you could be included in the final list from the Regional Office.
โก๏ธ Avoid filling out the form twice or thrice to prevent name duplication, which could lead to your name being removed from the final list.
2 For Senior High School, make sure that your guardian is not also a student. Your guardian must have a different name and should be living with you to be able to claim the assistance on the day of release.
3 Prepare IDs or proof of enrollment for this school year (e.g., Certificate of Enrollment, Study Load, or School ID). This will be required to confirm your eligibility.
4 Fill out the form before 12:00 NOON of MARCH 24, 2025. Late submissions will not be accepted. Be responsible and submit on time!
๐ As long as you fill out the form correctly, and youโll be on the final list then you will probably receive assistance from the LGU Sumilao.
