Php300 each. Down payment in any amount is required.
You can send your payment through Gcash -09558917741(Shayne Anne Louise Fernandez)or in cash, just see any of the officers in the SC Office, second floor, CTE Building.

For those who will be sending the payment through GCASH, send also the screenshot for confirmation. NO SCREENSHOT SENT MEANS NO PAYMENT RECEIVED.

If you are not sure of your sizes, you can get your sizes measured at the SC office, starting on Monday.

Fill up and submit this form for you to signify that you are enrolling this first semester of school year 2022-2023.

Please input your information in ALL CAPS. Incomplete or incorrect information will not be processed. Be sure to submit this form on or before September 7 (for old students, special students and transferees) and August 24 (for incoming freshmen) for us to process your enrollment.

For successful submission of this pre-enrollment form, you must receive an accomplished copy of this google form in the inbox (or spam folder) of the e-mail you will indicate below.

Note that if you are a regular student, you are no longer required to come to school as long as you successfully submit this form. But if you are irregular OR special student, you may come to school for enrollment (but you still need to submit this form to signify your intention of enrolling).
