For New Students:

1. Original Grade 12 Card (F138 / SF9)
2. Original Certificate of Good Moral Character
3. Photocopy of PSA/NSO Birth Certificate
4. Seven (7) pieces 1×1 photo (formal attire, white background)
5. Original Student Permanent Record (F137 / SF10)

For Transferees:

1. Original Certificate of Grades / Transcript of Records or any official academic record (for course evaluation)
2. Original Certificate of Transfer Credentials (to be accomplished by the FCAT Registrar)
3. Original Certificate of Good Moral Character
4. Photocopy of PSA/NSO Birth Certificate
5. Seven (7) pieces 1×1 photo (formal attire, white background)
