2. If your medical clinic does not appear in the dropdown list, please select “Others.” The information will be updated accordingly.

4. 𝗔𝗹𝘁𝗲𝗿𝗻𝗮𝘁𝗶𝘃𝗲 𝗟𝗲𝗮𝗿𝗻𝗶𝗻𝗴 𝗦𝘆𝘀𝘁𝗲𝗺 (𝗔𝗟𝗦) and 𝗧𝗘𝗦𝗗𝗔 should 𝗡𝗢𝗧 be written as your school's name. Indicate the actual school or institution where you completed the program.
5. 𝗡𝗖 𝗜𝗜 𝗶𝘀 𝗻𝗼𝘁 𝗲𝗾𝘂𝗶𝘃𝗮𝗹𝗲𝗻𝘁 𝘁𝗼 𝗮 𝘃𝗼𝗰𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲.
6. 𝗔 𝘃𝗼𝗰𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲 𝗶𝘀 𝗻𝗼𝘁 𝗰𝗼𝗻𝘀𝗶𝗱𝗲𝗿𝗲𝗱 𝗮 𝗰𝗼𝗹𝗹𝗲𝗴𝗲 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲.
