Early Registration for School Year 2026-2027
will start on January 31 to February 27, 2026.

To all:

• Incoming Grade 7 Learners (currently enrolled in Grade 6)

• Incoming Grade 11 Learners (currently enrolled in Grade 10)

•Transferees: Learners from other schools who wish to transfer to SUM-AG NHS

• Balik-Aral: Learners who stopped schooling for a year or more and wish to return

•REQUIREMENTS:
1. Accomplished Basic Education Enrollment Form
2. Photocopy of PSA Birth Certificate
(Alternatives, if PSA is unavailable: NSO Birth Certificate, Local Civil Registry (LCR) Copy, Barangay Certification, or Late Registration Document)
3. Photocopy of Report Card — for transferees and Balik-Aral learners only.

📣📣ATTENTION TO ALL INCOMING GRADE 11!

Here's the Enrolment Flow for Grade 11 on May 19- 23, 2025.

Importante Reminders:

📌 Please bring your parents/guardian during enrolment.
📌 Have a proper haircut, no hair color and earrings for all male students.
📌 Wear white t-shirt.
📌 Complete all the requirements for the smooth flow of enrolment.
📌 All Transferees, Repeater, Balik-Aral should have an evaluation first from the Office of the Registrar on June 9-13, 2025.
📌 Before proceeding to the enrolment venue, we will have an orientation for the new or pilot implementation of Strengthened Curriculum of Senior High School at School Gymnasium on May 19, 2025 at 7:30 A.M. Attendance will be checked.
📌 Enrolment Venue will be the SHS- Cookery Laboratory.
📌 Enrolment will start at 8:00 A.M. to 12:00 NN and 1:00 to 4:00 P.M.

Step 1:
📍 Grooming Inspection
The teachers will inspect the haircut and will give the enrolment form, the enrolment slip and will sign at the back portion to proceed to next step.
📍 Fill-out all the information of the Basic Enrolment Form, Track Form, have the slip signed by the parents and proceed to next step.

Step 2:
📍 Submit all the requirements to the Enrolling Officer, have the slip signed and proceed to the Step 3.

Step 3: BMI (Height and Weight)
📍 Take your height and weight.
📍 Have a health assessment.
📍 Have the parents signed the deworming consent form.
📍 The assigned teacher will sign the slip to proceed to the next step.

Step 4: ID Picture Taking
📍 Get ready with the picture taking or do with your best look, no heavy make-up.
📍 No payment for the picture taking.
📍 Have your slip signed by the assigned teacher after the pictorial.

Step 5: Confirmation of Enrolment
📍 Go back to Step 2 and have the slip signed by Grade Level Adviser (Mrs. Toga).

NEXT STEPS: JUNE 9, 2025

Step 5: Brigada Services
📍 The teacher will assign the students and parents for the areas to be clean.
📍 After the Brigada, have the slip signed by the assigned teacher.

Step 6:
📍 Keep the Enrolment Slip safely and this will be submitted to your Section Counselor during the opening of classes.
📍 Please wait for the call/text of your respective Section Counselors.
