Welcome to PUP Alfonso Campus!

As part of the enrollment and student health clearance requirements, all incoming freshmen are required to accomplish and submit the following documents:

✅ Chest X-Ray Official Result
✅ Medical Certificate (issued by a licensed physician indicating that you are physically fit to study or enroll in your chosen program)
✅ Health Information Form for Students (attach a recent 2×2 colored ID picture and accomplish Part I ONLY)
✅ Dental Health Record Form (the student should fill out the required information, while the last part must be completed and signed by a licensed dentist)
✅ Student Medical Waiver Form
✅ One (1) Long Brown Envelope (Blank)

📌 IMPORTANT REMINDERS:
• Print all forms on A4-size paper.
• Fill out all forms completely and accurately.
• Use clear and legible handwriting.
• Ensure all required signatures are affixed.
• Keep both photocopy and digital PDF copies of all submitted documents for your records.

The 𝐒𝐭𝐮𝐝𝐞𝐧𝐭 𝐀𝐥𝐥𝐢𝐚𝐧𝐜𝐞 𝐚𝐧𝐝 𝐆𝐮𝐢𝐥𝐝 𝐨𝐟 𝐄𝐝𝐮𝐜𝐚𝐭𝐨𝐫𝐬 (𝐒𝐀𝐆𝐄) warmly invites all SAGE students to participate in our upcoming Seminar and General Assembly, followed by the high-energy BrainBuzz Tournament and Games! The event will take place on June 11, 2026, at the PUP Alfonso Campus Covered Court, with the excitement kicking off at 8:00 AM onwards.

To help you prepare, here is the full lineup of activities for the day:

𝐌𝐨𝐫𝐧𝐢𝐧𝐠 𝐒𝐞𝐬𝐬𝐢𝐨𝐧 (8:00 AM - 12:00 NN): We will begin with the official registration, followed by an inspiring seminar entitled “What It Really Takes to Be an Educator”. We will wrap up the morning with the presentation of the organizational accomplishment and financial report.

𝗔𝗳𝘁𝗲𝗿𝗻𝗼𝗼𝗻 𝗦𝗲𝘀𝘀𝗶𝗼𝗻 (12:00 NN - 5:00 PM): The afternoon will be completely dedicated to fun and interactive activities!

We also have some important reminders regarding certificates, groupings, and your attire:

𝗖𝗲𝗿𝘁𝗶𝗳𝗶𝗰𝗮𝘁𝗲 𝗗𝗶𝘀𝘁𝗿𝗶𝗯𝘂𝘁𝗶𝗼𝗻: All students who attend the seminar on June 11, 2026, will receive a Certificate of Participation. Additionally, the certificates for the first two seminars conducted under the Seminar Series will be distributed on the same day. Please note that the issuance of certificates will be based on the official attendance records gathered during the previous seminars and the upcoming June 11 seminar. Therefore, students who have attended all three seminars in the series and attend this upcoming event will receive a total of three (3) certificates!

𝗢𝗳𝗳𝗶𝗰𝗶𝗮𝗹 𝗗𝗿𝗲𝘀𝘀 𝗖𝗼𝗱𝗲: Please be mindful of the dress code changes throughout the day. For the morning session, everyone is required to wear ANYTHING BLUE. For the afternoon session, you will need to change into your RESPECTED GROUP COLOR for the games.
