Blood Gas Analysis (ABG) – Quick Gists
Proper specimen handling is VERY important in Blood Gas Analysis because pH, pCO₂, and pO₂ can change quickly after collection.
📌 Specimen Collection
• Use a heparinized syringe
• Remove air bubbles immediately
• Mix sample well after collection
• Transport to the lab ASAP
Sample Stability
• Stable for 30 minutes at room temperature
• Stable for 1 hour if placed on ice
❌ Rejection Criteria
• Delayed transport beyond stability time
• Clots
• Air bubbles in syringe

Cells continue metabolism after collection:

• Increased acidity → pH decreases

• pH < 7.2 or > 7.6
• pO₂ < 50 mmHg
• pCO₂ > 55 mmHg
Small pre-analytical errors can lead to major clinical impact in ABG interpretation.

🧪 ASCP Quick Note: HIL Interference in Clinical Chemistry
HIL = Hemolysis • Icteric • Lipemic
These are the major sample interferences in clinical chemistry that may affect analytical accuracy and patient results.
🔴 Hemolysis
• Caused by rupture of red blood cells
• Releases intracellular contents into serum/plasma
• May falsely increase:
→ Potassium (K⁺)
→ LDH
→ AST
🟡 Icteric
• Due to high bilirubin levels
• Serum appears dark yellow
• Bilirubin may interfere with photometric reactions and absorbance readings.
⚪ Lipemic
• Caused by high lipid/triglyceride levels
• Serum appears milky or cloudy
• Can affect light transmission and electrolyte measurements.
⚠️ Always assess sample quality before analysis to ensure accurate laboratory results.
