Medical Preparation
Visit a Healthcare Provider for a Pre-conception check-up:

PREPARATION TIPS FOR LABOUR

Labour also called CHILDBIRTH is the process of your baby leaving your womb as at when due.

And it should be expected at 36-42weeks.

The basic LAWS of the Labour room are:

NO PANT
ONLY GOWN😒
OBEY YOUR MIDWIFE/DOCTOR!!🙏

WHEN YOU ARE CLOSE TO LABOUR,

Your baby drops into your pelvis, ready to come out... This is that point when you feel a little relief from all the baby's weight and even shortness of breath.

We call this LIGHTENING.!!💃💃💃

It can happen a few weeks or even just a few hours before your labour begins.

Also your baby will be moving less than usual.

YOU'LL KNOW YOU’RE TRULY IN LABOR WHEN:::---------

1. Uterine Contractions
You have contractions which is when the muscles of your womb tighten up and then relax...

You will feel your abdomen get hard... and then...get soft when the pain stops.

These contractions may feel like EXTREME PERIOD CRAMPS...

Your contractions are there to push out your baby.😍

The time between your contractions will keep reducing until they get so STRONG and REGULAR that you can’t walk or talk during them.😆😨

Not all contractions mean you’re in labor oooo.

You may experience what we call FALSE LABOUR or BRAXTON-HICKS CONTRACTIONS…

It happens sometimes when you feel the sharp pain come… and then go…

It can however be hard to tell the difference between true labour and false labour.

But note that in FALSE LABOR, the pains are irregular, and doesn't increase in consistentcy.
And most importantly... There's no Dilatation of the CERVIX.....

PAIN... PAIN...PAIN!!
Of course, you will feel pain in your belly and lower back that doesn't go away even when you move or change positions😂.

"SHOW".... MUCUS PLUG.

You will have a kind of bloody or brownish mucus discharge coming out of your va**na...

Jelly like...sticky... Azin it feels like Okro soup ....this is called SHOW💦

SHOW is a sign that your cervix is about to start opening for your baby to pass through...

SHOW is blood + mucus that seals your cervix during pregnancy to prevent infections from reaching your baby while still in the womb.

This mucus dislodges when labour is about to start and mix with blood to form the "Okro soup" you feel in between your legs😝

If you're losing more blood, it may be a sign that something is wrong, so rush to the hospital 🏥

RUPTURE OF MEMBRANE
The water💧 that protects your baby in your womb BREAKS!.

You may feel a big rush of water run down your legs OR you may feel just a trickle...

Once you arrive at the hospital, you will hear your midwife will ask you "Has your water broke"?

When it breaks at home, PLEASE use a sanitary pad but never a Tampon or Menstrual cup.

Sometimes, this water may not break at home, we might even break it for you at the hospital to speed up labour... Because once water pours out, you are closer to seeing your baby.💃

If your baby does not arrive within 24 hours after your water breaks, you'll be needing an INDUCTION OF LABOUR because, the water that protects your baby is out and so the baby now have an increased risk of infection.

CERVICAL DILATATION
Your cervix opens slowly starting from 2cm till its fully dilated which is 10cm.

URGE TO BEAR DOWN
As Labor progresses, you will also feel this urge to go to the toilet…

We midwives keep hearing “NURSE AH WANT TO POO” 😂…this is caused by your baby's head pressing on your retum.

We won't allow you visit the toilet oo🤨🤨 poo while pushing u hear?😆 don't worry your Midwives also have a degree in packing sh*t💩🤣

Labor is divided into 4 stages:

1. EARLY OR LATENT PHASE OF LABOUR...

Your midwife/doctor will tell you after putting her hand into your va**na, "Madam You are 1cm OR 2cm OR 3cm OR 4cm Dilated"

This is that stage when you will see a woman in labour taking a walk... From one end of the hospital to the other....resting and continuing....and you think she is doing drama baa🙄

She has been asked to exercise small for labour to progress a bit faster.

2. ACTIVE PHASE OF LABOUR

This is the main business of the day 😂

Your cervix dilates from 4cm - 10cm and your baby pops out!😁🤸🤸💃💃💃💃

There is something called CROWNING during this phase.

This is when your baby's head is visible at the vulva.....in an attempt to deliver the Head😫😫

This moment is EXTREMELY PAINFUL😣... like a burning sensation and we refer to it as RING OF FIRE!🔥🔥🔥🔥🔥🔥🔥🔥🔥🔥🔥🔥🔥

We also guide your Perineum so you'll gently deliver the Head and the remaining part of your baby to prevent your va**nal skin from tearing on it's own due to overstretching...

OR

You receiving a cut from the midwife's scissors called EPISIOTOMY😊...

At this stage of Labor, you'll hear EVERY Midwife and Doctor screaming PUSH!!!

Wondering why?🙄

THIS IS THE MAJOR MOMENT OF LIFE AND DEATH!

Your job however is to relax and OBEY your midwife!

PUSH WHEN YOU ARE ASKED TO...

When pushing, don't just scream Ma🙄...

Use this EXPO👇

Hold your two ankles...
Look at your navel and....
Try pushing out the midwife's finger in your va**na, your baby will follow!☺

3. DELIVERY OF THE PLACENTA

If any problem occurs at this stage, there's problem!🙆

We born Baby, we never born Placenta? Everybody wee start sweating ooo😥

While cleaning, resuscitating(if need be) and dressing up your baby, we wait for the placenta to separate before delivering it...

You will feel another round of slight contractions at this point BUT we no longer need your pushing Ma.😋

4. RECOVERY

We monitor you and your tiny bundle of joy for any wahala like bleeding...

You will be required to start breastfeeding immediately too🤱

Once you are stable, you are good to go over to Postnatal ward for further monitoring.

REMEMBER!!!!!......
Its OK to have a different Labour experience from what your friend or neighbor felt during her own childbirth.

To every expectant Father seeing this, have a tangible gift ready for you wife once your baby arrives.... Childbirth Is Not A Child's Play!

THIRD TREMISTER OF PREGNANCY
The most beautiful moment that every one is always waiting for
Boom the pregnancy is now showing up the tummy is big and beautiful; the back takes a c shape written in opposite direction. The mother can now clearly feel the fetal movements. In most circumstances it’s the preparation stage.
Third trimester of pregnancy starts from 28 weeks to 40 weeks but in most cases, it ends when the baby is born
From 36 weeks the baby is considered term meaning the baby has already matured and can survive outside the uterus
Signs and symptoms of third trimester
• The abdomen is enlarged and the pregnancy can now be seen.
• Easily fatigability due to weight gain.
• Shortness of breaths especially when sleeping on a flat surface but gets better when sleeping in sitting up position.
• Frequency In micturition (too many movements to the wash room) This happens because the growing baby in the uterus exacts pressure on the bladder and hence the frequency in urination.
• The feet may be swollen from the ankle joint. this mainly happens due to weight gain extra fluid retention and hormonal changes. This should not be a reason to panic unless your blood. pressure is high more than 140/70 taken at rest or when urine test is done there are presence of protein in urine.
• Itchiness on the belly due to stretch marks. The skin around the abdomen stretches and dries up. a moistening cream can really be of help.
• Braxton Hicks contractions also known as false labour may happen as the baby turns to take a new position
Third trimester checklist
Kindly you should ensure you are well prepared for the baby. The following are some of the preparations;
• Individual birth plan. Choose the facility to deliver in and ensure you check on affordability and the preparedness of the facility set up in case of emergencies.
• Means of transport to the facility. Please avoid last minute rash as this will or may delay you from reaching the health center.
• You should also prepare for a birth companion and inform them in advance. It’s very advisable to have your spouse present as this will help in taking care of the baby.
• Baby clothes and individual personals should be purchased and packed in a bag waiting for the final day.in baby bag ensure after your last clinic you put all ultrasounds and the antenatal booklet and place it in an accessible area when need arises.

SECOND TRIMESTER OF PREGNANCY
This is the most amazing moment and most feel like announcing to the whole word that they are pregnant
It’s from 13 week to 27 weeks
The second trimester of pregnancy often brings a renewed sense of well-being. The worst of the nausea has usually passed, and your baby isn't big enough to make you too uncomfortable. Yet more pregnancy symptoms are on the horizon.
• Growing belly and breasts. The belly will enlarge as the baby grows the breast will enlarge too and become sensitive
• Braxton Hicks contractions. Also known as false labour.They usually occur in the evening after work or exercises and sometimes after s*x .The lower abdomen becomes tight and a sharp pain Is felt often it does last for 1 minutes it’s a preparation for labour
• Skin changes. This is due to hormonal changes the skin becomes hyper pigmented
Dry, itchy skin, especially on the belly
Increased sensitivity to the sun. This means you might burn more easily. Make sure you wear a strong sunscreen when spending time outside.
A dark line (“linea nigra”) down the middle of your belly from your navel to your p***c hair
Patches of darkened skin on the face (sometimes called the “mask of pregnancy”)

• Dizziness. This is mainly due cardiovascular changes
• Leg cramps this is due to the growing baby pressing on the leg nerves and blood vessels that supply to the leg. Always ensure you lie on side and not on the back
• Dental issues. It’s caused by hormones the ligaments loosens and the gums becomes sensitive.it usually resolves after pregnancy
• Nasal problems. It’s as a result of increased blood supply
• Vaginal discharge.
• Urinary tract infections the growing baby presses the bladder and hence during urination some urine is retained. Always take plenty of water
Heartburn may begin or worsen in the second trimester. Your growing uterus presses on your stomach, which can force food and acid up into your esophagus, causing the burn.
DANGER SIGHNS
should be relatively smooth sailing for the next three months. However there are a few symptoms that do always warrant a call to your doctor, including heavy va**nal bleeding, severe abdominal pain and fever over 37.5 and above
Also keep an eye out for signs of gestational diabetes (which usually starts around week 24 to week 28 of pregnancy), including extreme thirst, frequent and copious urination, extreme fatigue and snoring. Also talk to your doctor if you notice sudden weight gain, severe swelling in the face and hands and vision changes, which can be signs of preeclampsia.
WHAT TO DO
• Do regular and routine antenatal checkups
• Pre genetic counselling
• Always perform minor exercise
• Heat well balanced diet as it will determine the baby preferences in life
• Do a scan from 16 weeks if you wish to know the baby gender it can be determined from this stage

First Trimester of Pregnancy

Term pregnancy is about 40 weeks. First trimester is from the time of conception up to 12 weeks (3 months)
Signs and symptoms of pregnancy
• tiredness
• upset stomach
• vomitting
• mood swings
• breasts tenderness
• heartburn
• weight gain
• headaches
• cravings for certain foods
• revulsion to certain foods
• constipation
Immediately you realize you are pregnant one is advised to start antenatal clinics and start taking folic acid.
Folic acid is very important during this trimester as most organs develop at this period
Health measures during this period
• Take prenatal vitamins.
• Exercise regularly.
• Work out your pelvic floor by doing Kegel exercises.
• Eat a diet high in fruits, vegetables, low-fat forms of protein, and fiber.
• Drink lots of water.
• Eat enough calories (about 300 calories more than normal).

What to avoid during first tremister
• strenuous exercise or strength training that could cause an injury to your stomach this may lead to miscarriage
• alcohol
• caffeine (no more than one cup of coffee or tea per day)
• smoking
• illegal drugs such as co***ne ma*****na etc
• raw fish or smoked seafood
• shark, swordfish, mackerel, or white snapper fish (they have high levels of mercury)
• raw sprouts
• cat litter, which can carry a parasitic disease called toxoplasmosis
• unpasteurized milk or other dairy products
• deli meats or hot dogs

The Baby at 4 Weeks
At 4 weeks, your baby is developing:
• The nervous system (brain and spinal cord) has begun to form.
• The heart begins to form.
• Arm and leg buds begin to develop.
• Your baby is now an embryo and 1/25 of an inch long.
The Baby at 8 Weeks
At 8 weeks, the embryo begins to develop into a fetus. Fetal development is apparent:
• All major organs have begun to form.
• The baby's heart begins to beat.
• The arms and legs grow longer.
• Fingers and toes have begun to form.
• S*x organs develop
• The face begins to develop features.
• The umbilical cord is clearly visible.
• At the end of 8 weeks, your baby is a fetus, and is nearly 1 inch long, weighing less than ⅛ of an ounce.
The Baby at 12 Weeks
The end of the first trimester is at about week 12, at this point in your baby's development:
• The nerves and muscles begin to work together. Your baby can make a fist.
• The external s*x organs show if your baby is a boy or girl.
• Eyelids close to protect the developing eyes. They will not open again until

• young (below 19 years though it differs with countries)

investigations required during first trimester
• ultrasound to confirm the pregnancy
• perform a Pap test
• blood pressure
• test for s*xually transmitted infections, HIV, and hepatitis
• estimate your date of delivery or “due date,” which is around 266 days from the first day of your last period
• screen for risk factors like anemia
• check thyroid levels
• check your weight

Molar pregnancy
A molar pregnancy (also called a hydatidiform mole or HM) is a tumor (growth) that develops in the uterus (womb) at the beginning of pregnancy. In a molar pregnancy, the placenta doesn’t form correctly. Instead it develops into a mass of cysts (small sacs filled with fluid) this leads to lack of supply of the baby with food and oxygen like it does in a healthy pregnancy.
causes of molar pregnancy?
Molar pregnancy is caused when there’s a problem with an embryo’s chromosomes

There are two kinds of molar pregnancy:
Complete molar pregnancy. In this condition, there’s no embryo or placental tissue. It happens when chromosomes from the mother’s egg are lost or not working, and chromosomes from the father are copied, so all 46 chromosomes come from the father.
Partial molar pregnancy. In this condition, there’s an embryo and maybe some placental tissue. The embryo may start to grow, but it can’t survive. This condition happens when the embryo has two sets of chromosomes from the father and one from the mother, so it has 69 chromosomes instead of 46. This can happen when the father’s chromosomes are copied or when two s***m fertilize one egg.

Signs and symptoms
• Faster than usual growth of the uterus
• Vaginal bleeding or a dark discharge from the va**na in early pregnancy
• Nausea and vomiting, which may be severe
• Passage of grapelike tissue from the va**na
• Absence of foetal movement or heartbeat
• High blood pressure
• Hyperthyroidism – an overproduction of thyroid hormones, which can lead to weight loss and increased appetite.

In most cases its detected through an ultrasound

ECTOPIC PREGNANCY

An ectopic pregnancy is a pregnancy that grows outside the inner lining of the uterus. The Fallopian tubes are the most common locations for an ectopic pregnancy.

It occurs when a fertilized egg lodges in a Fallopian tube or other location instead of continuing its journey to the uterus, where it is supposed to implant. The egg can become stuck when a Fallopian tube is damaged, scarred, or distorted.

SIGNS AND SYMPTOMS

1.Light va**nal bleeding and pelvic pain
2. stomach upsetand vomiting
3.Sharp abdominal cramps
4. Pain on one side of your body
5.Dizziness or weakness
6.Sharp Pain in your shoulder, neck, or re**um

Causes of ectopic pregnancy
1. pelvic inflammatory disease (PID)
2.Smoke ci******es
3. Older than 35
4.Have a s*xually transmitted infection
5.Have scarring from pelvic surgery
6.Had a previous ectopic pregnancy
7.Tried to have tubal ligation (tubes tied) or tubal ligation reversal (outdated but can occure)
8.Use fertility drugs
9.Had fertility treatments such as in vitro fertilization (IVF)
10.It could also happen if you become pregnant while you have an intrauterine device (IUD) for birth control.

Treatment
Mainly its surgical intervention when it has ruptured or medication can be used in early stages before rupture
